If you have any special health problems, have your doctor prepare your medical history for a foreign doctor. It should include your name and address, social security number, insurance company name and address, name and address of person to notify in case of emergency, blood type, medical history, current medications and dosages (generic names), list of drug allergies, reasons for prior hospitalizations, list of immunizations and dates.
